The
form we use for the order form is to be filled out and then printed by
you. The information typed into the spaces will not leave your computer or
be collected by the form in any way.
The form is a
PDF file. Most likely your computer can already read a PDF file. If not
click the "Get Adobe Reader" button below. This is a free download.
No Printer?
If you are not connected to a printer review the questions asked on the
form and write the answers on a sheet of paper that you will then include
with your tapes or movies. Please print clearly.
Return
Shipping : This is the address you would like to have the tapes/ films
and CDs/DVDs returned to. You will notice that this group also asked for
phone and email. Please include here the phone and email of the person
being contacted. This is the phone or email we will contact when tapes
arrive.
Billing
Address: This address is for billing purposes. This address will be
used when processing your credit card. If it is the same as return
shipping address just click the box the reads, "Same as Above". If you are
not going to pay with a credit card it is not necessary to fill this
section out.
Free
screening Option: Our free screening program is for those who are not
sure if they really want their tapes transferred or not. We only offer
this for audio services. Not video or movie transfer services.
There are 2 boxes to choose from: 1) Just transfer I know the content
of my tapes (faster service). Or 2) Please create an mp3 sample page and
contact me. With the first choice we process your tapes and transfer them.
This is for those who have no doubt that they want their tapes
transferred. The second choice is for those who might have found the tapes
in a family members attic or closet and have no idea what is on them. They
may contain family members talking long ago! Or they might contain
Grandpa's old bluegrass collection, which, no offence to Grandpa, they do
not want.
If you send tapes to be screened and decide you do not want the audio
they contain simply pay to have the tapes returned or ask us to keep them
and pay nothing at all.
Copies:
"How many copies of each CD would you like?" This is pretty straight
forward. You can order additional copies of your newly made CDs for just
$5.00 each. So if you are having a tape transferred and know that other
family members are going to want a copy for themselves, this is where you
let us know. If you don' want copies just leave blank. (The original CD
being made does not count as a "copy". So, if you want to end up with a
total of 2 CDs you would want to order "1" copy.
Label:
Here we want to know your decision on label printing. The drop-down box
presents the choices talked about in
Step 4 or our step by step.
Just drop-down the menu and click the option of your choice. Visit Step 4
for more information. If you are providing the text for the label use the
box to the right of the drop-down menu labeled "Use space below for "Text
Label".
Payment:
Next we ask about how you plan to pay. We also collect credit card info
here. Remember this form is not being sent electronically. It is only
collecting your info for the purpose of printing a completed order form.
The first option is Check. Select this if you are paying with a
check. You don't have to actually enclose the check. This just lets us
know you plan on paying with a check. Make checks payable to "Reel 2 Reel
2 CD".
Next we have credit card options. Type of card, number, cardholder
name and verification number. This is a 3 digit number on the back of MC
and VISA and a 4 digit number on an American Express.
Special
Instructions: Lastly we have left a space at the far right near the
top for you to type any special information you would like to share with
us.
